Older adults living with HIV face growing challenges in managing their health as they age, despite successful treatment that keeps the virus under control, according to new research from South Korea.

The study, published in BMC Public Health, involved interviews with 12 individuals aged 50 and older, who had lived with HIV for nearly two decades on average. While all maintained undetectable viral loads by adhering to their medication, the participants struggled with other health issues related to aging.

Many were diagnosed with chronic illnesses such as diabetes, high blood pressure, and dental problems. Yet, they encountered barriers when seeking treatment outside infectious disease clinics. Some hospitals refused surgeries after learning the patients were HIV-positive, forcing them to rely on university hospitals even for minor ailments like nasal allergies.

Mental health was a major concern. Participants reported feelings of isolation and difficulty recognizing their own mental health needs. They also struggled to find proper counseling, as many professionals lacked understanding of HIV or the needs of sexual minorities.

Social isolation was widespread, with 83% of participants living alone and experiencing shrinking social networks. Many were excluded from family and friend circles. The study also cited previous findings that nearly half of people with HIV in Korea have not disclosed their status to others.

Long-term care options for people living with HIV remain scarce, with only one or two facilities available, described as poorly maintained. Participants feared developing dementia or disabilities that would threaten their independence.

The researchers concluded that while HIV treatment is stable, older adults with HIV face new and complex health challenges. These include physical, mental, and social difficulties that require comprehensive and coordinated support.
